We are reaching out to community partners, letting them know San Diego PACE is actively accepting referrals during these trying times.

We are in the midst of the COVID-19 pandemic that may leave our elderly feeling alone as they shelter in place.  SYHealth offers several programs that support the most vulnerable community members to stay healthy in the community safely.  Please know you can be confident in our efforts to continue operations and to assist individuals in need of our services during this time.  San Diego PACE is currently conducting screenings over the phone for the safety of our elderly population and our staff.

How Can You Help?  

Please refer persons who appear to meet the criteria:

To make it as easy as possible for you to refer those who can benefit from our program, you can send individual’s name and phone number directly to me via e-mail or fax their information utilizing the attached Referral Form. 

San Diego PACE is a comprehensive solution focused on keeping seniors 55 and older stay healthy and live safely in the community. We also prevent hospital and long term care admissions through the delivery of high quality compassionate care. San Diego PACE offers free transportation services to all medical appointments and an array of services delivered or offered in the home such as meals, medications, supplies and home care which are critical for seniors during this time of need. At this time, we have implemented telephonic behavioral health counseling for seniors enrolled in our program and work closely with families to offer much needed support and information.
